Next in the "Modern Architecture" category is the Provo 5th and 8th Ward Chapel, which has some great exterior elements that I enjoyed looking at. It is located at 200 S 500 E in Provo.
I talked with some members in the ward who appreciated the architecture but also noted that it's a very small building (and parking lot) to be housing 3 wards.
I am working on a personal history of my great grandfather. Back in the 1950's he would have attended church in the Provo 5th and 8th Ward building. Looking at your description you say it is at 200 S 500. I think you meant 200 N. This was a great gem to come across.

On May 3rd, 1953 President Oaks blessed his baby in this chapel. It was there first born baby Sharmon Oaks. It was the first baby ever blessed in the brand new chapel. That insight was taken from his recent autobiography.
